Proposal Form

A Proposal form is the most important and basic document required for insurance contract between the insured and insurance company.
It includes the insured's fundamental information like postal address, age, name, occupation, person’s medical history, details of the asset to be insured, location, claims information and insurance history etc.
This information is necessary since it assist the insurance company to assess nature of the risk to be insured.
The proposal form should be filled truthfully by the person taking insurance since it forms the basis of the insurance contract.
The proposal form ends with a declaration which must be signed and dated by the proposer.
